Attention Washington State Government Workers: We are in network with MetLife Vision only - not Davis Vision by MetLife or EyeMed Vision Care.

We are widely in network with the options listed below - coverage for every plan varies, please confirm with your insurance carrier that we are an in-network provider for your specific plan, and what your coverage benefits are before scheduling an appointment. Vision insurance is only applicable for your annual eye exam and glasses/contact lenses. All other appointments will be either billed to medical insurance or out of pocket.
